These forms only need to be completed once a year, so Fall or Winter Athletics participants do not need to complete them.
All High School student athletes are required by the IHSAA to have a completed physical, and it is encouraged for Middle School athletes to complete one as well. If your child has not turned in the completed form to the Athletic Office, please schedule a physical immediately. You will need to take the IHSAA physical form to the appointment so that the physician may complete and sign it.
