These forms only need to be completed once a year, so Fall or Winter Athletics participants do not need to complete them.
All student athletes are required by the IHSAA to have a completed physical. If your child has not turned in the completed form to the Athletic Office, please schedule a physical immediately. You will need to take the IHSAA physical form to the appointment so that the physician may complete and sign it.

ISI Staff
Led by strong senior leadership and a tight-knit team, the Gryphons worked hard all spring, and it paid off! Evan Johnson advanced to regionals with a school-record 75, and the team set a new school record with a 328 at sectionals. With dedication, talent, and a unified team spirit, this year’s Boys Varsity Golf team made their mark on ISI athletics and set a high standard for future seasons.
Coach Bio
About coach Jared Johnson
Jared Johnson is originally from Carmel, Indiana. He brings a diverse athletic background to his coaching, with a strong foundation in basketball from his time with the Carmel High School program in the late 1990s. Over the past decade, he has developed a deep passion for endurance sports, actively competing in cycling criteriums, time trials, and marathon running since 2010.
Coach Johnson has been part of the ISI coaching staff for five years. His coaching philosophy centers on developing each athlete to their fullest potential, physically and mentally. He focuses not only on improving athletic performance, but also on building resilience and life skills that extend far beyond the playing field.
