The COVID-19 Liability Release Form must be signed and submitted by December 13 in order to return to in-person learning in January. A separate form must be filled out for each individual student. Families who refuse to sign will be required to participate in the remote learning services.

Additionally, the following forms are required for those wishing to register and/or participate in the relevant ISI programs in alignment with guidelines for maintaining the health and safety of our school community during the COVID-19 pandemic. 

Please note that registration/participation in some programs require additional fees for service. The registration deadline for all forms listed below is July 28. 

Child Care & Upper School Extended Supervision Registration: sign up for child care and/or extended supervision for Semester 1

Monthly Menu Request: students, faculty, and staff  who wish to select alternative lunch menu options must submit requests monthly

Request for Remote Learning: request remote learning services for the first quarter of the upcoming school year
