The following forms are required for families who wish to register and/or participate in ISI programs in alignment with guidelines for maintaining the health and safety of our school community. 

Child Care & Extended Supervision: all ISI families may register for services per session, semester, or for the entire academic year 

Enrichment ProgramLower School families may register for the third session of enrichment classes December 1-15

Spring AthleticsUpper School families may register to participate in the spring athletics season


 

Individuals 5 and up are now eligible to receive the COVID-19 vaccine. ISI strongly recommends COVID-19 vaccination for the continued protection of our students, faculty, staff, and community. Per our Student, Faculty, or Staff Illness policy individuals who are fully vaccinated may be afforded fewer restrictions, specifically with required quarantine procedures. 
For the purposes of this policy, fully vaccinated individuals have received all doses of vaccines, including the booster if eligible. People are considered fully vaccinated for COVID-19 ≥2 weeks after they have received the second dose in a two-dose series (Pfizer-BioNTech or Moderna), or ≥2 weeks after they have received a single-dose vaccine (Johnson and Johnson [J&J]/Janssen); individuals 12 and older who received the two dose series of Pfizer-BioNTech or Moderna vaccines are eligible for a booster 6 months after completing the primary COVID-19 vaccination series.Individuals who received the Johnson and Johnson [J&J]/Janssen single dose vaccine are eligible for a Pfizer-BioNTech or Moderna booster at least 2 months after receiving the J&J/Janssen COVID-19 vaccination.
Once your child is fully vaccinated your student’s vaccination card will be uploaded to Open Apply. Please do not upload until both doses in the two-dose series have been completed and are reflected on your student’s vaccination card.  
If your child is eligible (age 5 or older) and will not be vaccinated this school year, please let the school know, by completing the following form. This information is critical to our ability to make decisions about continued COVID-19 policies and procedures, as well as quarantine instructions.
